The belly button is a scar, not an organ. It is what is left where the umbilical cord was cut, and its shape is decided by how that healed rather than by anything you did. Two completely different operations get called belly button surgery, and mixing them up is the most consequential mistake on this subject.
One is repair of an umbilical hernia, which is a gap in the abdominal wall behind the navel that lets tissue push through. That is a medical problem with a surgical repair. The other is umbilicoplasty, which reshapes the skin of the navel and changes nothing underneath. Which one you need is settled by whether there is a defect in the wall, not by how the navel looks from outside.
An outie you have had since you were a baby is almost always just how the cord separated, and it is nothing to treat. A bulge that appeared in adult life is a different story, because the usual reason a navel starts protruding later is that the wall behind it has given way. Pregnancy, weight gain, heavy lifting, chronic cough and fluid in the abdomen all raise the pressure that opens that gap.
The test people can do themselves is simple. Lie down flat: a hernia usually flattens or disappears and returns when you stand or strain, while a navel that is simply shaped that way does not change. That single observation is what a consultation should start from, and any clinic that quotes you for cosmetic reshaping without asking it has not examined you.
If the whole abdominal wall is loose after pregnancy or weight loss, see tummy tuck. After major weight loss, including weight loss following gastric sleeve, the navel is usually addressed inside a larger operation such as a fleur-de-lis tummy tuck rather than on its own.
The surgeon opens over the navel, pushes the contents back inside and closes the gap in the abdominal wall. A small defect can sometimes be closed with stitches alone. Most adult hernias are repaired with a mesh, because suture-only repairs of anything but the smallest defects come back more often. It can be done open or by keyhole, and the choice depends on the size, on previous surgery and on the surgeon.
Two things surprise people afterwards. The first is that the navel may look different from before, because the skin has to be tacked back down to the repaired wall and the old shape is not always recoverable. The second is that recurrence is a real possibility, and it rises with obesity, smoking, heavy lifting and anything that keeps abdominal pressure high. Ask your surgeon what they do about those before the operation rather than after.
Umbilicoplasty reshapes the skin of the navel. It can turn a protruding or stretched navel into a small vertical slit, tighten a hooded upper rim, or narrow one that has widened. The scar is placed inside the rim so that it sits in shadow, and that placement is the whole craft of the operation.
What it does not do is change the abdominal wall. It does not flatten a stomach, it does not tighten loose skin above or below the navel, and it does not repair a hernia. If the wall underneath is intact and the only thing you want changed is the shape of the dimple, this is the operation. If any of the other three things is what is actually bothering you, this operation will disappoint you.
A tummy tuck lifts the entire skin envelope, so the original navel stays attached to the abdominal wall and is brought out through a new opening in the relocated skin. The navel you have afterwards is therefore a rebuilt navel with a scar around its rim, and its shape is one of the most visible signs of how well the operation was done.
That is why the shape of the navel is worth asking about before an abdominoplasty rather than trying to correct it afterwards. If you are considering both, ask the surgeon to explain how they position and shape it, and ask to see it in their healed photographs at a year rather than at a month.
